Sizes differ in more than toe length; Elite most comfortable
I'm writing this review for those of you who may fall between sizes. As an 8.5 I was stumped because of prior experience with another DJO boot, so I ordered a small Elite and medium Standard, swayed by pricing. I completely concur that the Elite is a more comfortable boot. If you can afford it, go for the upgrade. Strictly speaking, the small is a tad short in length (if you are an 8 it will fit perfectly) but overall it fits much better than a medium in my case (5' 5", small boned, narrow feet and calves), and since it is an open-toed boot I can adjust the padding. The plastic shell cover will amply protect my big toe. The medium is tall enough that it interferes at the knee with my talking a step; I have a much more natural gait with the small. The medium is also so much wider that my ankle is not as well supported, even with inflation I have a lot more internal movement; my heel rises up as well. The small is snug enough that I don't even have to inflate it. And this is with a currently swollen ankle two weeks post surgery. I imagine the differences would become even greater as swelling subsides.

Helped Significantly to Bring Down Swelling
This Aircast helped to significantly bring down the swelling from a Grade 3 ankle sprain and is helping to provide stability, protection, and support while my ankle heals. I wished I had purchased it sooner than I did.

I have the short medium aircast. I’m a true size 8.5 narrow women’s shoe. I have a 5th metatarsal bone bruise and fractured pinkie toe. I’m allowed to weight bear. I broke my pinkie toe by running into a wrought iron gate. I’ve broken a toe before, but this felt different and kept swelling even a month after the injury. Long story short, the dr thinks I have a bone bruise on my 5th metatarsal, and apparently if you walk around on a weakened bone that’s bruised it can break (stress fractures.) I have two toddlers and can only sit for about 2.4 minutes before somebody needs something. In order to avoid the flexing action, dr said I need to wear a short boot when walking around, and no long walks for me. I mentioned how I bought my medical shoe on amazon, and the dr suggested I check to see the dr office price first since I have insurance. I spent $80 on this on amazon. In the Drs office WITH insurance it would’ve been approximately $225 ???? I like that that an aircast can be removed. I would get super claustrophobic in a regular cast on my foot. I’m allowed to walk without using crutches. It’s not the lightest thing ever and can get uncomfortable, but when I do get to sit I can take it off and stretch my ankle around to keep it from getting too stiff. This version of the cast is slightly more $$, but it has two air pumps integrated into the boot so you don’t need an extra device. This is crucial because the boot fits differently when sitting vs walking. I’m constantly readjusting the pumps, so it would be annoying to have to carry a pumping device with me. The Velcro straps secure it pretty well, and there’s plenty of padding inside. The footbed is okay. It’s completely flat but I think for a foot fracture that’s probably better. If you have ankle problems maybe ask your dr if you can put a support in there. The picture for this listing is INACCURATE. The foot shield that goes on top has two additional pieces that extend toward the back of the boot under where the top strap secures. When I got the boot, this part was tucked inside, and at first glance that’s how the picture looks, so I assumed that’s how it goes. Luckily I looked up videos after only a few days of wearing and instructions because my shin was killing me from the strap being tight (enough to support my foot) and digging in. Turns out those pieces go on top and prevent the shield and strap from being too tight on your shin. Tips: -watch videos if your doctor didn’t fit you. -if your foot feels like it’s burning on the bottom when you wear this, try wearing a sock. The foam created a weird friction for me. -readjust the top of foot strap and air pumps when you’re sitting. And again when you stand. It doesn’t fit the same. -buy a shoe that is raised for the other foot (they also sell something you can put on your regular shoes.) I measured about a 1.75” difference. Ehh, not quite as hoped
I was annoyed that the promised next day delivery didn't happen. I waited 2 extra days for this. AMZ wasn’t the cheapest option, but I thought I would get it faster… While this boot is better than the (incorrect!) boot I was issued at the hospital post-surgery, I feel like my foot wants to supinate in this boot (I am a women’s 7). As I am to wear this for weeks, I don’t want to develop bad mechanics, so be prepared to add other support measures per your comfort and your doctor’s consult. I found the top armored piece put too much pressure on my swollen foot no matter how loose I adjusted the velcro, so have been using without. Don’t hesitate to modify as needed! The air pump/bladders help with fit, but only at the achilles/ankle bones.

8 Months in an Air Cast - THIS IS THE BEST BOOT ON THE MARKET!! Nothing Compares!
I had to wear a cast for 8 months. In that time, I went through 3 different boot casts- and not for comfort but for durability. Long story short, this cast costs more, but you will be in way better shape mentally and physically. Better shock absorption, better design, better internal airbag system. Through and through this is the best walking boot on the market, and well worth the value. Don't compromise on this- I went through two cheaper boots and still ended up buying this and wishing I did from the start. I didnt know a boot cast could be this comfortable, with both previous boots causing callouses and hot spots with extended use. This cast was so comfortable I actually started to enjoy walking around again.

Excellent quality & worth the money. Helped me to become more mobile & quicker, than using the cumbersome 'ski boot' that the NHS give you. Far more comfortable & much much lighter. Offered more support too as able to adjust the air pockets to suit your foot/ankle size

Not for someone with collapsing arch!
I wish this could have worked for me but it made my foot problems worse. I have posterior tibial tendon dysfunction my arch has collapsed so much I went from a size 9 to a size 11 in shoe size… I also happen to have something wrong with my Achilles’ tendon aswell and if I’m on my feet for to long I get severe edema. I also have over pronation.. all of these problems are in both feet :( I would’ve liked them had I been able to find inserts specifically for the cast to support my arch rather then push my feet down on the flat surface. Just returned them waiting for refund then I’ll be purchasing the aircast pttd braces. Hoping to get some relief as I don’t have a family doctor and er doctors have been useless… although it didn’t work for me it did seem to be a decent quality product but it was a bit on the pricey side
